For New Patients

New patients should call the office for their initial appointment.

Please bring with you any lab work you have, or other health records you feel are important.  You may also email these to office@integratedholistic.com and/or fax them to (847)-251-0066

Also, please bring any current medications, supplements, herbs, protein powders, etc.  If you can not physically bring them, please bring a written list.
